Not very familiar with Paul Horn, as he seems to have made detours into mysticism after promising jazz beginnings.

I'm sure you have already spotted, and probably dismissed, the one potential candidate I can see,

Are you sure you're not getting confused with "The Story of a Jazz Musician" by David Wolper, which isn't a book, it's a short (26 mins) documentary film about Paul Horn.
